Form 8822 - Change of Address

General Information

Form 8822 is used to notify the IRS that you have changed your name and/or address.

Name Change

If you (or your spouse) changed your name, be sure to complete line 5a(or 5b). Also, notify the Social Security Administration of your new name.

Information about Spouse

If you are married filing jointly (i.e., your filing status on line 3a of the Background Worksheet is 2), we automatically carry information about your spouse from the Background Worksheet to Form 8822, lines 4a and 4b. Otherwise, we set those lines blank.

If You Move After Your Return Is Filed

If you changed your address after your return was filed (especially if you expect a refund), be sure to notify your local post office of the change, too.
